Informed Consent

Provision of Services
You understand that I Dont Do Addictive Drugs offers a variety of clinical services to its clients including: consultations, short term individual counseling (up to 12 sessions), long term counseling, couples therapy, psychiatric services (in some locations only), crisis intervention, group counseling, case management, and referral. During your initial assessment, a counselor and you will work together to determine how best to meet your needs. you further understand that appropriate referrals will be provided to you if it is determined that you would be best served by a different resource.

Nature of Counseling
you understand that there may be both risks and benefits associated with counseling. Risks may include experiencing uncomfortable feelings, such as sadness, guilt, anxiety, anger, frustration, loneliness, and helplessness, because the process often requires discussing the unpleasant aspects of life. However, counseling may improve your ability to relate to others as well as provide a better understanding of yourself, you values, and an improved ability to deal with stressors. Although counseling can be beneficial, it may not be helpful for everyone.

Counseling Staff  
I Dont Do Addictive Drugs is staffed with licensed and supervised psychologists, counselors, social workers, psychiatrists, psychiatric residents, graduate interns, and pre-doctoral psychology interns. Interns and residents work under the supervision of licensed professionals.

Confidentiality
You understand that I Dont Do Addictive Drugs counselors maintain confidentiality in accordance with the ethical guidelines and legal requirements of their profession. Effective counseling sometimes requires that staff members share confidential information with other staff members. Note: Staff often consult about clients and share clinical records of clients whom they are treating.

You understand that no records or information about you will be released from I Dont Do Addictive Drugs without my consent, except under the following circumstances:

  • When doing so is necessary to protect clients or someone else from imminent physical and/or life-threatening harm.
  • When a client lacks the capacity or refuses to care for him/herself and such lack of self-care presents substantial threat to his or her well-being.
  • When the abuse, neglect, or exploitation of a child, elder adult, or dependent adult is suspected.
  • When a client pursues civil or criminal legal action against the Center or its staff, or when a client makes a complaint to a Professional Board about a clinician.
  • When a client is involved in a legal proceeding and there is a court order for the release of records, or when a release is otherwise required by law.

Conflict of Interest
Clients who plan to apply for training opportunities at I Dont Do Addictive Drugs  are discouraged from receiving individual or group therapy at I Dont Do Addictive Drugs due to potential multiple role conflicts. Students who want additional information about this policy and community counseling resources should talk with the Director/Clinical Director.

Records
Your records are stored electronically and include the information you provided and information about any interactions  (counseling, phone calls, consultation, emails, etc.) with I Dont Do Addictive Drugs staff. This information is only accessible within I Dont Do Addictive Drugs and is protected by multiple security measures. This information may be de-identified and used for research or quality assurance purposes.
EMERGENCY
If you are experiencing a psychological emergency that is life threatening, call 911 or go to your nearest emergency room.  In the event that you are experiencing a psychological emergency that is not life threatening, please contact the center at (760) 607-6429
Consent
You certify that you have read, understand, and agree to abide by the information outlined above regarding your eligibility and use of I Dont Do Addictive Drugs Services. you hereby give your consent to authorize I Dont Do Addictive Drugs to evaluate, treat, and/or refer you to others as needed. You will have the opportunity to discuss any questions regarding the above information with our intake staff before you sign the document at our office.
